Supported Decision Making

Ohio Network for Innovation's Decision Making Support Team wants to help you get support to understand your choices and how your decisions might impact your life, or get help to communicate your decisions to others.

At ONI, we understand that no one's needs are exactly the same, and not everyone needs support in the same way. We offer a variety of services across a spectrum of types of support. From less restrictive Supported Decision Making (SDM) services, to Power of Attorney, to more restrictive supports through our Guardianship services.

Informal Supports

ONI's Decision Making Support Team understands the importance of making your own decisions. Our team is set to help you make your own choices, no matter what they may be.

Examples:

Supported Decision Making for finances, legal matters, work, and/or everyday life.

Formal Supports

Sometimes you may want someone you trust to make some of your decisions for you. You can decide who this person is and what kinds of decisions they make for you.

Whether it’s an emergency, immediate, short-term, or future need that can be planned for, our unique approach to guardianship eliminates waiting lists, allows for collaboration with community partners, and offers reliable and person-centered solutions.

ONI uses a variety of resources and tools to guide our Decision Making team at all levels of support. These resources help guide conversations and facilitate Supportive Decision Making. You can preview some of these tools here

ONI strives to find whatever the least restrictive means of support, often that comes in the form of Supported Decision Making. We use a variety of tools and resources to help guide/facilitate those conversations.

Sometimes, you don't just need support making informed decisions, you need/want to choose to have someone you trust make certain decisions on your behalf. You choose who this person is, and decide what decisions they make for you. 

Examples of these types of supports include:

- Power of Attorney

- Chosen Rep.

- Conservatorship

These supports are a bit more restrictive as they require (temporarity, or until you decide to terminate the arrangement) signing away some of your legal rights to make decisions for yourself
