Formal Supports

Sometimes you may want someone you trust to make some of your decisions for you. With formal supports, you can decide who this person is and what kinds of decisions they make for you. Formal Supports can include things like a Chosen Representative, a Power of Attorney, and a Conservator.

Types of Formal Supports

  • Chosen Representative

    • Choose someone to (help) make decisions about your I/DD services​

  • Authorized Representative​

    • Choose someone to (help) make decisions about your Medicaid​

  • Power of Attorney​

    • Choose someone to (help) make decisions about your healthcare and/or finances​

  • Conservatorship​

    • Choose someone to (help) make decisions of any kind and ask Probate Court to oversee this​
